Flights to BGW, KBL, etc are very popular, driving from KWI or AMM to Baghdad isn't a fun way in to the country (less so when the Yanks were shooting everything that moved), or so my colleagues tell me. More popular driving that route than driving to KBL from ISB though.
Oneworld (and often the alliances in general) do have a bit of a "hole" in it's coverage in certain countries. BMI has plugged the gap a little, but Iraq, Afghanistan, Iran, Pakistan, Somalia, Burma, Cuba, Syria, The Stans spring to mind as places where milage accrual is harder that you'd expect.
