Delta's international tariffs provide for ground transportation between LGA and JFK (and vice versa) on a paid, international itinerary.
Not on domestic itinerary? I had asked about ground transport between LGA and JFK in Skymiles forum. The answer I got was that Delta privodes ground tansport between LGA and JFK. Domestic itinetararies were not excepted in the answer.
From Delta's domestic tariff:

"RULE 30: INTER-AIRPORT TRANSPORTATION

When a metropolitan area is served by more than one airport and the passenger requires
connecting service with arrival at one airport and departure from another airport, transportation
between those airports must be arranged by and at the expense of the passenger. Baggage
must be claimed and rechecked by the passenger."
