I tried to double check all the meeting points by comparing to Jason's pdf -- it looks good. But

On Krakow - very well done - congrats
On Warsaw - good locations as well! I'd definitely add
- "Uprising Museum"
- "Szucha" Museum (Szucha 25)
- "Pawiak Museum"
- Ghetto Trail (pretty long walk)
And would rather skip: Railway Museum, Firefighting Museum, Main Library.
Looking at the other locations in Poland - pretty diverse interest list! On Concentration camps: unless you have some personal reasons to visit some of them, I'd strongly recommend visiting Auschwitz and then deciding if you want to see more.
