At the beginning of the year, I ended up with a package with the virtual provider Red Bull Mobile. It uses the network of a provider called Play, which is a real price fighter.

The starter package included a few gigabytes of data (it said so with very big letters on the package) and came with two cans of Red Bull for 50 zł IIRC (€ 12.50 / US$ 15).

It was available at the petrol station across the street from the Warsaw Hilton (along with all the other major networks) and you will find many outlets selling all the major networks (don't expect any advice though from the sales people unless you go to a proper phone store). The coverage of Play in Greater Warsaw was perfect, and I suspect it will be the same in Krakow (but not necessarily in between if you travel by train). The package came with a normal SIM, not a micro (of course you could cut it to size or have it done at most any phone store).

No identification at all is needed, you can buy with cash if you want to be truly anonymous.
