I had it happen once several years ago on a Delta Connection flight (don't recall which carrier it was). I later mentioned it to a neighbor that is an FAA inflight inspector and was scolded for not reporting it. She was correct, I should have reported it. Most major carriers take safety as a serious issue, but they can't address a problem if they don't know about it.
Happened to me, too, once on a different airline. But my seatmate was a close friend...and an FAA Principal Inspector. I didn't have to report it.
