American living in Spain a long time. Of your choices, I would go with Spain BUT do realize the bottom could drop out at any time. Also we find many Spaniards to be provincial in attitudes and certainly not what one would expect of a 1st world country; Racism as well--sat with university colleagues watching USA-Spain playing basketball two nights ago and was shocked at what they were openly calling the Americans "monos" (monkeys) "negros de mierda" (sh*t n*ggers) etc.
And no, the health system is not good, it has been hit or miss with us when we were using the public hospitals. I highly recommend you purchase a private plan through MAPFRE or whatnot for your coverage if you choose Spain.
