Dead easy, I did this last year, though didnt end up using it really. If you call OnePass and explain they will give you a fax number of the dept that deals with this. I think I faxed my Aeroplan statement and copy of my card. Not much effort. Check about timing though so that you get the rest of this year and next rather than just a few months. I got compd to gold with AC Elite. Ferrari has flown CO quite a bit I think so might offer more info on the program.
