Here's the breakdown of the open-jaw fare in WT+ (October 12 midweek):

Code:
Fare 1: Carrier BA TKX3UKJB LON to YVR
Passenger type ADT, round trip fare, booking code T
Covers LHR-YVR (Premium economy)
	£382.50
Fare 2: Carrier BA TKX3UKJB PHX to LON
Passenger type ADT, round trip fare, booking code T
Covers PHX-LHR (Premium economy)
	£339.00
BA YQ surcharge (YQ) £239.00
United Kingdom Air Passengers Duty (GB) £130.00
United Kingdom Passenger Service Charge (UB) £34.49
US International Departure Tax (US) £10.70
US September 11th Security Fee (AY) £1.60
US Passenger Facility Charge (XF) £2.90

Subtotal per passenger £1,139.69
Number of passengers x1

Total airfare & taxes £1,139.69
The only extras for CW would be the high fuel surcharge, which is GBP359, so an extra GBP120 per person.

As a matter of policy, BA seem to only waive the offline booking fee when the site is broken and not when a piece of functionality is just missing (by design or omission).
