I applied for the business card last week, but they only asked for one document to verify my address. I believe I was given the choice between a business certificate from the state, or a utility bill, although it may have been bank statement or utility bill. Either way, I asked twice and confirmed that they only needed one document.

Sent in a utility bill two days ago. Just called to confirm that it went through--was told it arrived and they would get back to me in 7-10 business days...
