Customer Appreciation Credit

Just got back for LHR to IAD. Paid C. They must have had to swap out the plane because no lie-flat.

I get an email from UA saying to go to the website and select what type of credit I want for my inconveneience. www.united.com/appreciation.

I fill in the details. I note that it says *don't click continue* unless I have 3-5 minutes to complete the rest of the form.

I believe I've done this in the past and I've been presented with options...miles, credits, etc.

When I hit continue all I get is this message? Something busted?

The response email I just got is even more puzzling.
