Domestic--If the airline is selling upgrades for Rs 1000-2000, and most people who want it can get it, the airline has too many business class seats.
International--Probably the same as above. If people are complaining about not getting an upgrade to business even when the business cabin is empty, means too many business class seats than it can sell.
May be the routes you travel on are different from the routes about which people complain.
