I don't quite see how AC would enforce the latter scenario? If my ticket is refundable with a fee, what's to stop me from refunding the ticket, forfeiting the refund fee, and purchasing a new ticket with a lower fare (assuming the fare difference is greater than the refund fee)?
The last line of yours says it all... Let's say you have a HKG-YVR-HKG refundable ticket (with a $200 refund penalty). You see the same trip adverstised a week later for $100 less. You cannot change your ticket as you loose the fare. Cancelling and rebooking means you are out $100. So you are better off to stay with your original purchase.
