So, BA lost four chances increase revenue/decrease Avios balance, as per my example above. Happier customer saving the cash/miles of course!

Sounds like it's a systems issue. The system wasn't built with the thought of what passengers could want (MFU at the airport). I'll ignore the catering issue for now!
