There are a couple of ways that confirmed upgrades can be lost. Off the top of my head two are equipment downgauges and the deadheading pilot (http://www.flyertalk.com/forum/air-c...-aircraft.html). It does take pretty extraordinary circumstances to lose a confirmed upgrade but it can happen.
