It appears that your friend bought the ticket through Expedia (bold is mine) and has either inadvertantly filled in the name boxes incorrectly during the purchase process, or Expedia had made a mistake.
Your friend should have contacted Expedia as soon as the ticket was issued to correct the mistake.
I have bought air tickets from Expedia before and the instruction was very clear to enter the Middle Name only if it is on the ID.
