I would guess they will treat you as a new member. One other pointer is that if you're eligible your referred will get their 12k very quickly. If they don't you can ask Amex or just cancel the card without bothering reaching the spend target.

To your other question, I think most of us would rather have the extra 2k Avios rather than £10 but it's your decision.
