I don't know the answer to any of your questions but I do know that domestic flights to STT are upgrade eligible if you prefer to gamble. I was just in SJU last week and I was upgraded on a T fare both ways and my lady friend, who is a GM, was upgraded at the GM window on an award ticket. My guess is STT would be the same.
