I don't think that is how it works, at least in the PMCO days for sure. I have not been in this situation since 3/3, so I don't know. IME, what happens is that the waitlist expires and it is transferred over to the airport waitlist, but you will still have instrument upgrade priority along with the inherited status of the account of the instrument owner.

No worries... The upgrade list does run automatically, just not instantaneously right after R space opens up. Between, the time R space opens up and the time they finally process the upgrade automatically [once a day] anybody can skip the queue by requesting an instrument upgrade or using miles. Also, close to departure, they do release saver award space the same time as R space, so people can book with miles during that time frame.
That has been the norm with CO, and now is the norm with the new UA, and it is a system functionality that I do not like. I hope they will modify the codes to have the system automatically run the upgrades every hour or something >24 hours out. Within 24 hours, the upgrades run everytime an upgrade eligible customer checks in.

I do not think that is true. It is sorted by status of the upgrade requestor and fare paid. I have never been in that situation though, so take my understanding of the system with a grain of salt.
