For example, on my driver's license, First name is Johan Smith and Last Name is Anderson,
My father's name is Smith.
But in school, work etc. I am known as Johan, people only use the first given name And my signature only contains the name Johan.
In passport, we don't have a field for middle names, only a field called (Surname) and a field called (Given name(s)).
So, what should I write in ESTA? I would like to be on the safe side

I've travelled on an ESTA and ticket that didn't match (I booked the ticket in my first + last and my ESTA was in first + middle + last, and I was asked to come to the gate to verify names, but it wasn't a major issue.)
I would probably put the same on the ESTA as you did on the ticket - who are they to tell you that your name isn't first + first + last? Johan Smith in first, then your last in last, leave middle blank again? I have a friend Heidi Lou, and that is her first name, she would never put 'Lou' as a middle name in a form.
