My apologies for being thick, but I am not able to figure out where MAD comes into your itinerary.

Could you please explain where it comes in? From your post it sounds like you are flying CAI-LHR in CW but where does MAD fit?

Or, are you actually flying CAI-MAD-LHR (but then "CW" does not fit into this scenario)?
