I went on to use GlobalCall (www.global-call.net) after finding AT&T Universal (or rather Citicorp) had raised rates to $2+ for domestic calls. Not the cheapest but better than AT&T. Their rates from the U.S. is quite good if you don't use a pay phone.

3rd country calls (e.g., Germany - Canada) would run $1 / min on average.

Also have 800 access numbers from a lot of countries.

Don't like their $1.49 monthly service charge.
