I was asked for the same thing when I applied. Can't remember exactly what I sent, but it was along of the lines of an electric bill and and a bank statement--both of which are personal. Was a bit nervous that this wouldn't be sufficient, but I was approved soon afterward. I applied for the business card as a sole proprietor with the business name as my full name.
