Do they hand out the cards for the premium immigration counters in BKK? The last couple of times I arrived BKK the immigration was a real zoo but luckily I could use the premium line. For some time now BKK immigration can take as long as 90-120 minutes.

Yes LX give the fast track immigration cards. Sometimes not stamped with airline/date so you may end up with a spare if not asked for it.
