Bulkhead does become available within T24 prior to the flight. At that point, I would suggest a quick phone call to reservations to snag those seats and if they aren't able to do that request to speak to a CRO. Mind you, even if they are able to get you into a bulkhead seat, you can still be removed to accommodate the two cases I mentioned above.
It's hit or miss depending on who takes your call. I've had success before.
I have DVT that led to a pulmonary embolism years ago. I'm in pain if I sit still for more than a couple hours at a time. I get to know the FA's in the back quite well because I'll spend as much time talking to them as possible.
These days I'll only book a flight if an exit row or EC seat is available.
