I made a booking (over the phone) using household points and my Amex 241 last year for myself and a friend (who isn't part of my household account). Does it make a difference that I was flying too perhaps?
On a 241 you can take someone not in your HHA. In other situations you cannot use points for anyone outside the household.
From the Amex 241 rules:
20.Companions do not have to be people who are named within a British Airways Executive Club Household Account. Member’s can choose to take anyone as a Companion on their Companion Voucher booking.
