For $200pp look at places like Fleur de Lys for example, it is $72 for a three course menu, $82 for four course and $95 for five course and that is before tax, tip or drinks.
The top rated places to look at are Ame, Coi, Benu, Saison, Atlier Crenn, La Folie, Commis (Oakland), Bix, Piperade, Quince, Michael Mina. Gary Danko, Farallon, Acquerello, Masa’s, Spruce, Aziza and the mother of fine dining in CA Cuisine, Chez Panisse in Berkeley..
Just fun, traditional SF places, check out the Tadich Grill, it is the oldest restaurant in the state, it goes back to the Gold Rush days, Sam's Grill is just a few years behind it, Swan Oyster Depot just behind them and then Alfred's for steak.
I don't think SF has a great sushi place. I know that is LA snobbery, but I also don't think LA has the same number of high quality, high end places SF does. LA really shines in ethnic food.
