We should not assume that just because BA refused to take Poxball's money that they are losing out. Revenue management is not as simple as that and if we get to the point where people expect to at-airport upgrades, then I'd say BA are the ones that would be losing out.
In Poxball's case, I doubt this was an op-up because it happened on board. Of course it's possible that the seat turned out to be broken etc - quite a different matter again from an op-up in any case.
The case I had was definitely involved an op-up though. I tried to part with the money to upgrade from CW to F and there was nothing loaded into their system so they couldn't, and there was definitely an op-up from CW to F.
My comment relates more to the cases reported where people have tried to pay to upgrade but there was no facility in the system yet op-upped happened - mine is not an isolated case. There are even reported cases where people tried to pay to upgrade and failed, yet they ended up being op-upped. That's definitely lost revenue.
