You book thru the airline's website. I use matrix to search then head over to the airline website. If not bookable online, you can call, and 99.9% of the time they will waive the phone booking charge as the itinerary wasn't bookable online. As to your last question, you don't. I mean, you could look at fare basises (or bases? haha) but largely there's no clearcut way to know you're at rock bottom.
