This happened to me on my last phone booking. The person who took my booking was utterly useless. He managed both to make some serious errors in the reservation (which eventually got fixed after a lot of hassle), as well as to enter my email address and other details incorrectly.
The booking was an Avios one done online and I didn't enter my email address at all. It picked it up from my account and emailed me my booking confirmation correctly. It's just this subsequent communication that's gone wrong.
I think the most likely thing is that there's some manual intervention in cases where flights are cancelled and someone probably looks at the proposed alternatives and then does something that triggers an email to the customer. Perhaps they enter the email address again at that stage and just got it wrong? It's the only explanation I can come up with at the moment.
