The smallest unit is the Everest3 without the battery pack. I had one thinking battery power usage for travel but the battery pack is not covered by medical insurance and it was not cheap.

I use the medical tag bought here for my separate CPAP bag.
Mine was self made business card size that I got laminated using clip-arts from the public domain.
