+1000 to noise-cancelling headphones or sound-isolating earphones.
I've even taken to wearing the eyeshade (depending on where I'm sitting). I didn't think it would make a difference, but it does.
Slightly OT: ever see someone put the blanket over their head while sleeping during a flight? I don't think I could do that.
