This is why Japan has a huge supplemental insurance industry. That 30% can add up to be a pretty huge sum, a lot of which is simply the cost of occupying the hospital bed for however ridiculously long period the doctor prescribes.
The base room at my hospital was free after insurance, and the fee for food and clothing was nominal. I paid an extra 2625 yen a night to upgrade to a room with only two beds (and that bed ended up being vacant half of the time)
One of the guys who stayed in that room said that he tries to avoid checking out for as long as possible, because it is cheaper to stay admitted than to live at home.
