There is no link or other special code in the mailer.

It just say three ways to apply:

fill out the application by mail which has her name and address preprinted.

offer is 50K miles with $69 annual fee and cardmember anniversary bonus of 3000 miles (which my signature is $99/6K).

I think they keep the file open for 28 days so I might wait two weeks and have her call and see if they can reconsider. Otherwise wait six months and reapply and hope the current offer is around in some form.
