CO actually spells out the subtleties on their lounge access page: "Puerto Rico and the U.S. Virgin Islands are considered international for the purpose of accessing United Clubs, but may not be considered eligible for entry into US Airways Clubs."
Dunno how Guam counts. They used to be explicit about it, iirc.
