I use the UK Train Times iPhone app from Agant Ltd, and I've been pleasantly surprised. When I arrive at Paddington and select "Next train home", it tells me which platform to go to and where to change. I've discovered that the platform number appears on the app before it appears on the destination boards, so you can get to the platform before all those people staring up at the boards. Another good feature is that it'll display a list of the stations at which the train you're on is stopping, with scheduled time and ETA at each station. I'm no fan of UK trains (being forced to put up with First Great Western has given me low expectations), but this app is surprisingly good.
