My wife and I were waitlisted on a flight today, and didn't end up getting upgraded. Not a big deal, until I noticed that there were 2 free adjacent seats in the J cabin.

Is this common, that there are waitlisted customers who don't get upgraded even while there are open seats? Is it usually a case of a no-show or something like that, or would it happen for other reasons?
