Bulkhead can (and is) assigned by special services (forget the name of the desk - any res agent can queue your PNR to them) far in advance for those with handicaps that qualify and call. I suppose that could apply to row behind as well, but have generally seen bulkhead pre-assigned.
And I'd not be surprised if CK agents can as well.

I was under the impression that certain rows on every aircraft are blocked until the gate, even for elites. For example, Row 10, 11, and 17 are blocked on the 763. And, those seats are never for sale to non-elites. But, once in a while, when I look at seat maps from expertflyer, I see that someone is able to select one of the "blocked" seats weeks or months in advance. As you can see, someone is able to select 11B, an otherwise blocked seat.
So, how in the world do you select a blocked seat?
