Southwest was my thought too. There is no change fee, so you can cancel and apply the full value towards a future trip. Anytime and Business Select fares are fully refundable according to their web site too, and are probably much more reasonable than United's refundable fares.

If you dont mind flying WN after the re-schedule, I recommend the "Wanna Get Away" fares. They are generally cheaper than "Anytime" and "Business Select" fares but they cannot be refunded back to your card, but full original value is available for future use.
The other option you mentioned is cancellation during medical, this *may* work if your wife is on the PNR, but I'm not sure how UA sees a pregnancy as "Serious injury or illness". You definitely can satisfy all the paperwork requirements, but I recommend calling UA and asking.
