If we are talking about Costco I'm sure they get a great deal from Amex since that's the only CC they take. I doubt they pay anywhere near 3.5%. My business is a fraction of the size of Costco and I only pay 1.9% for Amex. Usually businesses pay the percentage on the purchase and again on the refund. The store doesn't get the fees refunded they actually get charged again.
