People are allowed on planes with assistive orthopedics, oxygen concentrators and other medical devices. Belt extenders aren't only for the obese.

NECN reports the special equipment measured 21"W x 22"D x 31"H. Owing to his condition, Nicholas is significantly safer inside it than out of it once it's securely belted. There's no reason to suspect it wasn't securely belted on earlier flights.
