There have been a few occasions over the years that a safety issue has been overlooked or neglected that I felt it was necessary to file a DOT complaint or notify the FAA.

The FAA makes it pretty easy.

The fun part is that if you choose to not remain anonymous, the airline will respond very quickly and from the upper management level.

I had a flight a couple of weeks ago on a different AAirline. Issue was minors (clearly under 15years old) in exit row and no briefing by geriatric, aloof crew. Complaint filed on a Monday. Phone call from someone in the "Executive Offices" on Friday morning.
