My experience is the same: the *A rule is definitely more generous than the LH interpretation for their own mid-elites.
Years back I had the most bizarre rule rained upon me at the SEN cage in LHR. Was on the same flight as a lady who got bounced at by the LD as she was not eligible to use the lounge in the eyes of the latter.
When I tried to guest her, the LD explained that the rule is that "you have to be friends for some time, you cannot just host anyone".
After me asking for the detailed criteria for guests - if intercourse is required or if platonic encounters were permissible in LH's eyes - she made the usual "one time exception".
