In terms of ways that they might tap a wider audience, a card which guaranteed gold, diamond, or platinum status annually, even for a sizeable fee, may be worthwhile to certain customers. I have not seen the survey, nor do I know the Amex/HHonors inside details. I'm just looking at a potential market that could be tapped. HHonors Surpass only guarantees Gold Status for a year.
