The drive there was ok, exactly 1 hour with no major traffic, but I had to pay 81 (standard fare given by the Yellow Cab staff at LGA on the yellow paper slip) and a mysterious 30 USD surcharge that the cab driver told me as soon as I got in the cab and told him about the voucher. Tolls were about 3+12+6 or something just below 30 (I only recall it was below the last figure).

The cab driver was obviously very grumpy since on the receipt he just wrote down 21 USD for tolls + 4 USD for tip. I gave him 135, which is 45 out of my own pocket.

Once I arrived at LGA, the checkin agent did not see my reservation as it was not existant, luckily there was enough space on the place and I even got a free seat next to me in a fully booked plane (nice coincidence).

Is there anything that can be done in my case? If yes, what is the best way and which nuisance should I report? The delay? The added 45 USD?

There is no $30 surcharge and the tolls are $9.50-$12.50. The voucher includes the tip. You are absolutely entitled to compensation: from the cab driver. UACO has nothing to do with this, they gave you the voucher.
