Do you have the ticket number on your credit card bill? I know my UA ticket numbers appear right there. Also, on my credit card company's web site, it additionally lists the actual routing along with the ticket number.

I've called in the information to UA via phone right off the credit card statement before and they've been satisfied with that. Maybe the same strategy might work with LH.
