Evacuation insurance

I have just returned (actually still in the process) from a trip where I was the trip doctor on a private train in China to Russia.
Along the way I had a gentleman suffer a heart attack and I learned more about Chinese hospitals than I ever cared to know. During this time I tried assisting with his evacuation insurance. Having had another bad experience several years ago in the Caribbean with a Danish company I have a few questions to pose to the group.

1. What companies present the most and the least barriers to their clients when abroad and in need?

2. What experience has anyone had with the included evacuation insurance provided by the American Express Platinum Card?

3. What seems to get these companies to provide the coverage they have promised?
