I think this should be escalated to IHG management.
I had also quite often similar problems when booking rooms for a family of 3.
Most annoying for me is the missing
fixed and complete fineprint of all reservation confirmations, as most extra cost for child / extra bed are simply missing or details might be changed by manual input from IHG side / hotel or by IT problems.
I had an excellent experience with IC Amstel Amsterdam as AMB- they gave me on 2 SD nights an upgrade to exec suite (after leaving my email about child's stay in a room for 2 unanswered but were aware of the child during check-in).
On the other hand in IC Bali it was quite annoying to deal with a 13 years old child (see also post #496 at IC Bali thread

I can understand if there is a legal reason behind the booking limitation for 2 per room only, but most hotels would allow a child, at least if charing parents bed. This and also any additional costs should always be mentioned in the reservation fineprint.
The same information should be given when I can book (online) a room for 2 adults and 1 child (many hotels can accept 3 persons per room). I hate it to negotiate rates for bfast or extra bed afterwards.
