I get this sometimes (all the time) when a ticket is booked for me through a travel agent, I think it is because they are not linking my AE number with the ticket, so even though the name is the same, they just aren't connected.
*face palm*.... YEGUSER already explained this. You cannot add a FF number to an airmiles booking, period. That is the issue here.
