It only shows up if you are offered the US Airways MasterCard whilst you are in the process of buying a ticket.

You don't seem to actually have to buy a ticket; you just have to apply via the banner ad offering $50 off. Calling in to the bank and complaining the credit didn't show up may or may not work.
