You can purchase the airport limo on arrival @ HND, you can ask them for suggestion of the best way to get to your hotel using their service too. (which I did when staying at service apartments)

The ticket counter is just outside baggage claim. With a bright orange and white sign board.

Taxi is North of 4,000 yen to 10,000 yen would be my guess before 20% surcharge.

There are fixed fare cabs but I can't give you more information because I have never used them.
